What it checks
- File version
- fileSpecVersion must be 4.10.0. iQIES has rejected every other version since April 1, 2026.
- Header
- Facility ID (CCN), state code, report quarter 1–4, and a federal fiscal year.
- The 22.5-hour limit
- No Employee ID over 22.5 hours on any day, added up across all job codes.
- Codes
- Job title codes 1 to 40, and pay type 1 (exempt), 2 (non-exempt), or 3 (contract).
- Hours format
- Numbers from 0 to 22.5 with at most 2 decimals, like 7.55, never 7:33.
- Dates
- Real YYYY-MM-DD dates, all inside the quarter named in the header.
- Employee IDs
- 1 to 30 characters, only the characters CMS allows, and listed in the employees section.
- Replace files
- A warning when processType is "replace", which overwrites everything already submitted for the quarter.
What it can't tell you
- Whether CMS accepts the file. Only the iQIES Final File Validation Report says that.
- Whether the hours are right. It can't see your payroll or agency invoices.
- Whether old and new IDs for the same person are linked across vendors.
Built from the CMS PBJ Data Specifications V4.10.0 and the CMS business rules ArrivSure's own PBJ export checks. Free to use. Not affiliated with CMS.
Contract and agency hours usually take the most time. ArrivSure's healthcare module turns provider check-ins into signed-off PBJ entries and exports v4.10.0 XML.
